Use this form to request a change to a property address within the Town of Mountain View. Address changes may be requested by a property owner or initiated by the Town.
A $30 fee applies to address changes requested by a property owner. There is no fee for address changes initiated by the Town.
All requests are reviewed under the Town’s Address Change Policy, adopted by Council on May 19, 2025. Submitting this form does not guarantee approval.
